Significados atribuÃdos pelos avÃs no cuidado com a saÃde bucal dos netos: um enfoque qualitativo

RESUMO

The focus of the present study was early caries in childhood. This first part is structured by a theoretical reference chapter founded on a literature review addressing general aspects of the subject, such as family, grandparents, dental caries and health education. The second part consists of an original article on the subject. A systematic search of the literature was carried out on indexed scientific articles using health science descriptors in the Lilacs, Scielo, Medline and Capes databanks as well as books, dissertations and theses. The literature review evidenced the importance of studies using grandparents as references, researching elements that integrate various determinants in daily living regarding health practices adopted in strategies for promoting oral health in grandchildren. The aims of the original article were to identify the meanings given by grandparents in caring for the oral health of grandchildren, assess the dental conditions of the children through a clinical examination and determine whether the offering of sugar as a form of affection contributes toward early caries in childhood. A descriptive, exploratory, qualitative study was carried out based on the reports of nine grandparents between 38 and 70 years of age, guardians for grandchildren between 21 and 36 months of age and part of the early dental care program âGrowing up without cavitiesâ at the Areias General Hospital in the city of Recife (Brazil). Subjects were included based on the theoretic saturation criterion. Data collection was carried out from March to June 2007 through recorded interviews and clinical examinations for the detection of dental caries, visible plaque (biofilm) and white spots, using the descriptive analysis of the dmf-t and modified dmf-t index. The semi-structured interview contained four guiding questions, from which issues stemming from the corpus of the categorizations were extracted through content analysis of the cross-sectional thematic modality addressing the following: a) The meaning of care in relation to oral health; b) Grandparents: parents with sugar; c) Past experience with tooth loss; and d) Inter-generational conflicts affecting oral health. In the original article, it was concluded that there the grandparents studied attributed various meanings to oral healthcare. The understanding of the role they play in the family setting is important to promoting discussion and questions regarding attitudes and behavior in the control and prevention of dental caries, despite allowing their grandchildren to consume sugar as a transference of love and affection that is harmful to oral health, as proven by the clinical examination. The importance grandparents have as caregivers in childcare strategies after the first year of life is stressed with regard to the diagnosis of predictive factors of early caries in childhood and the establishment of prevention measures and health promotion
