Os fundamentos ideo-polÃticos das ConferÃncias Nacionais de SaÃde

RESUMO

This thesis is placed in the axis of the debates that have been taking place at the country regarding the participation of the different social segments, in the proposals spaces and deliberations of the health care politics, after the constitution of 1988. The object of this work is the political direction and the democratic foundations which are present in the proposal documents of the National Health Care Conferences starting from 1986. It is in this time that the debates of the democratization of the decision-making regarding health care services have formally transformed the conferences into privileged spaces, along with the Health Councils, in the three spheres of government. The Conferences and Councils are sometimes considered privileged moments of participation, where the interests conflict, and other times seen as spaces which make it possible to harmonize and guarantee the consensus among the different interests. Also, they are shown, theoretically and politically, as an opportunity for the socialization of politics and of the construction of another hegemony, articulated by the principles of the democracy. In the point of view of this work, the spaces of the Conferences, although formally delimited, are making it possible that many members of society and many public sector employees, penetrate the authoritarian and isolated borders that mark the history of the Brazilian public administration, being able to make propositions on the public health care politics. Even if, in fact, these instances have little altered the content and the form of the government politics, nevertheless, they are introducing new structures in the public-state ambit and, in a restricted level, are making it possible the socialization of information on projects and financing. Besides these elements that have outlined the analysis of the object of the thesis, I studied the SUS (Unified Health Care System) and the Conferences in the process going back to the origins of the democracy having as a basis the great events in the countryâs history and the determinations of the international conjuncture, above all in the phase of the structural crisis of the capital and its particularity in the Brazilian reality. In addition to that there is the understanding that health care is a right and that, included in the group of social rights, should be analyzed as a product of the historical development, marked by inequality and contradictions, which is linked to and reproduces with the totality of the relations between the capital and work. The hypothesis that orientated the elaboration of this study is that the Health Care Conferences, starting from the promulgation of the SUS and in the neo-liberal context of the decade of 1990, have been losing their democratic radical effect and their propositions are changing the principles of the SUS. This way, the emphasis in a public and universal system lives in a state of conflict and contradiction with the private-like focused model. The privileged materials for the research of the empiric material were the Final Reports of the 8th to the 12th National Health Care Conference. These documents are the main vehicle of popularization of the results of the plenaries, are as mediators between the Conference and the daily work of the politics formulation and are also the most complete expression of the group of actions that culminate in the accomplishment of the national phase of the Health Care Conference. In the analysis of the Reports I have found that the contradictions among the propositions are enormous. It seems that the debate is not directed to the dispute of the ideological and political direction and the form of organization of the services of health care. The prioritizations of specific demands take over and are above the collective projects. The disputes are for private projects and the depth of projects which could be socially benefic for the health care and the country has been lost. I understand that, in great measure, that these fifteen years of the SUS and the Conferences in its centre have represented either the option of following the easy-to-ride roads or an innovation with conservation, following the principles of the liberal democracy
