Laryngeal dysplasia is the precursor lesion to invasive squamous cell carcinoma. Dysplasia is an uncommon lesion; most cases is diagnosed as invasive squamous cell carcinoma. The criteria used to grade dysplasia in the uterine cervix are frequently applied in vocal cord dysplasia. However, there are histopathologic, pathogenetic and epidemiologic differences between intra-epithelial lesions in the cervix and larynx. The transitional epithelium in the vocal cord can cause diagnostic problems. The authors aim at characterizing severe dysplasia/carcinoma in situ of the vocal cord, evaluating its histopathologic and morphometric aspects (epithelial area and diameter of the nuclei), and proliferative index (ki-67), comparing the data with normal squamous and transitional epithelia. Among the 1400 cases of laryngeal carcinoma (1994-2006), 5 cases (0.35%) of severe dysplasia/carcinoma in situ were identified. The control group included 2 men who did not smoke or drink, and were 52 and 78 years old. The patients with dysplasia were predominantly of the male gender, in their 60s, and had smoking and drinking habits. When comparing dysplastic with normal and transitional epithelia, they noted that the former occupied a larger area in the epithelium, and had larger nuclei. The diameter of the nuclei per cell layer had significant differences in normal squamous epithelium , but not in dysplastic and transitional epithelia. The proliferative index was greater in dysplastic epithelium when compared to normal and transitional epithelia; staining was observed in the basal and parabasal nuclei in normal and transitional epithelia, whereas staining was observed in all layers in dysplastic epithelium
