Aspectos imunológicos e microbiológicos da candidose bucal. / Oral lesions of candidiasis are associated with microbiological and immunological features of oral candidiasis.

Oral candidiasis is a common disease and Candida albicans is the major infecting agent. C. albicans can be isolated from the saliva of healthy individuals. Innate immunity and the cell-mediated immune response participate in the control of fungal infections. The aim of this study was to correlate oral candidiasis with the number of colony forming unit (CFU) of Candida and to characterize the T cell response in patients with oral candidiasis. Participants included 75 patients. Two ml of saliva was collected from all patients for microbiological analysis. Cytokine levels were determined by ELISA in supernatants of peripheral blood mononuclear cells stimulated with C. albicans from the 25 patients with oral candidiasis. Oral candidiasis was observed in 48% of the patients and no association was observed with denture use. C. albicans was documented in the saliva of 91.7% of patients with oral candidiasis. There was an association between the number of CFU in the saliva and the presence of oral lesions. Erythematous and pseudomembranous candidiasis was more pronounced clinicals forms. Therefore a strong association between CFU and pseudomembranous candidiasis was found. A predominant type Th1 immune response was observed in supernatants of peripheral blood mononuclear cells stimulated with C. albicans. The mean SD of TNF- α was 327 310 pg/ml and of IFN-γ was 156 168 pg/ml. In contrast IL-5 and IL-10 levels were very low or undetectable. There was an association between number of colonies and oral lesions of candidiasis. Oral candidiasis is associated with a systemic immune response characterized by production of TNF-α and IFN-γ, which helps the host to control infection.
