Ãlceras de estase crÃnicas: aspectos clÃnicos e perfil bacteriolÃgico

RESUMO

The physiopathology of wounds do to chronic stasis of the lower limbs is multifatorial. Chronic venous insufficiency is the main cause. It occurs specially in women between the 6th or 7th decade. The chronic stasis wounds of lower limbs are the 14th cause of provisional license and the 32nd cause of permanent removal of work, becoming a serious problem of public health with a magnificent social and economical impact. The knowledge of association between chronic venous insufficiency and lymphatic disease in the formation and development of chronic stasis wounds of lower limbs is relevant to confirm the diagnosis and treatment. Seventy-three patients with chronic stasis ulcer were included in a prospective study. The sample was collected from patients assisted by the same physician which were spontaneously admitted in the vascular department of Hospital das ClÃnicas da Universidade Federal de Pernambuco (53 patients) And Santa Casa de MisericÃrdia de Santo Amaro (20 patients) in Recife-PE between April/05 and May/06. Age, race and gender were not considered. Patients with immunological disturbances, with associated diseases and in course of antimicrobial drugs were excluded. After anamnese, the ulcers were cleaned with saline solution (0.9%) and a wound sample culture was collected. Circulatory status of lower limbs was evaluated through Doppler velocity signals and duplex scan. The data processing were made by SPSS program (Statistical Package for the Social Sciences). In the sample, deep venous insufficiency was observed in 58.9%, 80.8% showed great and/or lesser saphenous venous insufficiency and 96% had perforator venous insufficiency. A significant association was verified between lymphatic disease and patients with advanced age (p= 0.027) and between lymphatic disease and time of disease in the lower limbs (p= 0.003). A significant association was detected between prior surgical treatment and patients with advanced age (p= 0.002). There was not the same significant association with time of disease in the lower limbs. There are no connections between lymphatic disease, chronic wounds and a specific bacteria species. A significant association was detected between deep venous insufficiency and time of disease of the lower limbs (p= 0.009) and did not happen with the age of the patient. The chronic stasis wounds of the lower limbs is developed by chronic venous insufficiency and lymphatic disease is associated with itâs chronicity
