Uma proposta de abordagem fisioterapeutica nas desordens da articulação temporomandibular

RESUMO

Temporomandibular joint (TMJ) is a highly specialized structure in craniomandibular complex. This structure can be compromised by neurological, orthopedic and muscle-skeletal problems, originating temporomandibular disorders (TMD). TMD carries a particular symptomatology, which can be difficult to diagnose and treat, accompanied by lack of coordination and painful manifestations associated to biomechanical unbalance of the joint and its related structures. Literature review and data collected from clinical observation led to the identification of the sites responsible for painful sensations in TMJ and their predisposing factors. A retrospective long-term study made it possible to evaluate the applicability of physical therapeutic interventions. Twenty-seven patients (22 women and 05 men; averaging 36 years-old, ranging from 15 to 56 years-old) were evaluated and treated with transcutaneous electrical nerve stimulation, ultrasound, heat, massage, joint mobilization, exercises, postural and general orientation regarding daily life activities. The follow up periods, ranging from 04 to 18 months after treatment, were carried out by a telephone interviews, in which the patients were asked questions related to the manifested complaints at that time. The results revealed 21 patients with no painful manifestations at the end of the therapy. During the follow up period, 16 patients were asymptomatic and 11 patients had remained in treatment by other specialties, as neurologists, dentists and others. The observed results indicate that an interdisciplinary approach is essential in these cases and a comprehensive methodology to physical therapy could play an important role in TMD treatment
