Progression of nutrition in patients with cleft palate with or without cleft lip. / "Evolução do estado nutricional de pacientes portadores de fissura de palato com ou sem fissura de lábio"

RESUMO

Introduction: the nutritionist role in educating families with cleft palate children becomes unnoticed once its role blends with general education from the other professionals of the interdisciplinary team that takes care of the children. Objectives: description of anthropometric progression in infants with cleft palate by an interdisciplinary team in a specialized outpatient setting emphasizing the nutritionist role in this context. Methods: forty–three patients with cleft palate or palate and lip met the criteria to participate in the study. The average age at the beginning was one month and five days, 24 were male and 19 female. It was taken anthropometric data (weigh, length, arm circumference, triceps fold, cephalic perimeter) upon admission. Later, those same measurements were taken prior to lip surgery (average at 8 months) and before palate surgery(average at 17 months). Z Scores of weight/age, height/age, weight/ height, were calculated, so as arm circumference and triceps fold. The tables of NCHS (1976) were used to calculate them. By recording 24 hours of a group of children with lip surgery and calculations of macronutrients and the amount of energy using Virtual Nutri software (Philippi et al., 1996) comparing with references of DRI/2002 it was elaborated. All education about the difficulties and preparing the food was done by the nutritionist, author of the study. Results and conclusions: upon admission more than 75% of the children were not breastfed. The amounts of calories and macronutrients ingested either pre or post lip surgery was appropriate. There was a significant increase of arm circumference and triceps fold between admission and lip pre-operation phase. The average values of Z scores of W/A, H/A and W/H didn`t change during the study, maintaining within the norms. Regardless social and economic difficulties all families overcome the obstacles throughout all phases of treatment, most likely due to the constant intervention from the interdisciplinary team, especially from the nutritionist who answered all questions from families involved.
