O manejo de questões da sexualidade emergentes na relação medico-paciente no setting da consulta ginecologica : um estudo clinico-qualitativo

RESUMO

This study s objective essence was the narrated gynaecologists experiences in the face of sexuality questions within the remit of a medical consultation. Our initial hypothesis was that there had been a certain modulated effective distancing, for the physician-patient relationship, established as a defence mechanism, with the purpose of being able to manage, in the most adequately possible manner, the sexuality questions that permeate a normal gynaecology consultation. In the face of this presupposition, our objective was to interpret the psychological significances attributed to such experiences and the diverse phenomena related by gynaecologists in relation to questions of sexuality, usually emerging within the medical consultation setting. For such the Clinical-Qualitative Method was utilized, which looks to discuss the relationships encountered between symbolic meanings and life experience phenomena. The subject sample consisted of medical gynaecologists, of both sexes, built up through the technique of data collection saturation, the normal strategy in qualitative investigation sampling. Consequently, in the field the number of people interviewed was defined. The data collection technique used in order to make the method viable was the Semi-Directed Interview with Open Questions, which allowed for in-depth contact between interviewer and interviewee, complemented by various field diary notes, and especially the careful and global observation of the informant. As to the procedures, the team made use of a Field Diary for noting data, divided into three parts: succinct identification data concerning the study s subjects; notes on the subject?s overall manifestations during the interview and self-observation data (reactions of the contra-transference type). The Data Treatment Technique used with the collected data was Qualitative Content Analysis, favouring categorization/sub-categorization, starting from the floating readings of the group of transcribed interviews ? the corpus. The Discussion/Interpretation of the results was conducted on the basis of psycho-dynamic concepts. The principal results consisted in the discussion of nine categories: 1) The medical consultation as a meeting within a dedicated professional bond; 2) The medical consultation as an identifying encounter; 3) The medical consultation goes beyond the bi-personnel meeting; 4) Peculiar contra-transference aspects of the gynaecology consultation; 5) Management of mobilized jumps in the relationship with a female; 6) Male ambivalent posture in front of a female; 7) Professional practice as a personal revision process; 8) Gender: some reflections about internal mobilizations in the face of the differences that make up men and women; 9) Concerning the non-availability of participating in the study. The conclusions point out that the physician-patient relationship is subject to the interference of emotional experiences that make up the psychism. The managing of awakened unconscious demands starting from the physician-patient relationship is associated with the capacity to deal with conflicting nuclei that permeate the dynamic field of human relations
