O Cuidado à criança prematura no domicílio / The care given to the premature at home

RESUMO

The caring of prematures at home generates concern by the professionals that supervise them until they are discharged from hospital, because there is no way to know how this caring is carried out and also what kind of support the families have. To understand this process it is necessary to consider the prematures specific needs and consequent differentiated care. Thus, the object of this study consists in the care of prematures at home. This research has as its main objective to analyze the care given to the premature at home and the following specific objectives: to identify the domestic care given by the mother or person in charge; to describe the care given to the premature and to describe the aspects that interfere in the process of caring for prematures. This is a qualitative study that had as its basis the human care emphasizing the premature care and the MMC method as attention strategy for low weight children. The study was accomplished in the homes of seven families of the county of Feira de Santana, who had a child in the MMC program of a public Maternity of the referred county from April to July 2007. To collect data, semi- structured interviews were used and also descriptive observation, which occurred in two different moments. The data analysis was made using the content analysis technique and was accomplished based on three thematic categories: the home caring of the premature, concerns that came from daily caring for the premature, and the social support for this care. The data collected informed that it is the mother who takes care of the premature and does so based on experiences and directions she received from the health team during internment. Also, she sees the child as fragile and vulnerable to interocurrence and uses the on time baby as reference for caring. Takes care of feeding using exclusively breastfeeding and is concerned about weaning. The mother adopts specific hygiene measures, heating and prevention of infections, vaccination of the premature follows different patterns. The care takers have concerns about the growth and development of the premature, as well as about the most common intercurrences. There are difficulties in monitoring the prematures due to the lack of structuring of the third stage of MMC. Concerning the social support, this study shows that the care takers need support for their task, and that the public policies should offer total attention to the premature after hospital discharge. This study is expected to become useful as basis for reflections and actions directed to change the reality of neonatal assistance and the effective participation of the nurse in all MMC phases.
