The nosocomial infection (NI) is a serious public health problem. The NI neonatology appears as a major concern of this issue, considering its magnitude and relevance. This research aimed to know the epidemiological pattern of hospital infection in newborns in the Neonatal Intensive Care Unit - NICU of a philanthropic hospital in the interior of Bahia in the period from January 2006 to December 2008. We developed a study of morbidity, a historical cohort (retrospective) of patients in a large hospital in Bahia. It was possible to rescue the number of NI by topography and patients to the three selected years (2006 - 2008) and describe the characteristics of cases in 2008. We collected data in a surveillance of NI and the medical records of hospital RNs in the study. Incidence Density of episodes was estimated of NI, using as denominator the number of patient-days. All rights of individuals and the institution have been respected, complied with the ethical principles. The data were processed using EXCEL spreadsheet program and analyzed with STATA and Epi Info 3.5.1 software. A total of 1984 RNs admitted to the neonatal ICU, representing 29 773 patient-days, 216 RNs whose NI incidence was 7,3 episodes per 1000 patient-days. In those RNs, 269 were recorded episodes of NI, which corresponded to an incidence of 9 episodes per 1000 patient-days. The most common incident topography was bloodstream infection without microbiological confirmation - clinical sepsis (with an incidence of 5,4 episodes per 1000 patient-days). Changes were observed with variation in the incidence of NI around the average for three years, showing reduced rates of HI in NICU during the study period. The incidence found was lower than the other studies, it is possible that the indicators of this unit are underestimated, and should consider the methodology for surveillance of nosocomial infections and the diagnostic criteria used by the service.
