RESUMO

Assess the impact of obesity treatment on adipokines, C-reactive protein and insulin sensitivity in hypertensive patients with central obesity. Methods: The study was realized through collected data and samples of stored serum from patients previously submitted to an open, analytical and controlled intervention study. Initially, there were sellected fifty one hypertensive patients, with age between 18 and 65 years; BMI ≥ 25 kg/m with fat centrally distributed. Patients were treated with orlistat 120 mg tid and hypocaloric diet or hypocaloric diet only, during 16 weeks. They were assessed in relation to blood pressure levels, anthropometric parameters, visceral fat, insulin resistance and sensitivity indexes, adipokines (adiponectin, leptin, IL-6 and TNF-α) and C-reactive protein (CRP) dosages. Results: after an average reduction of initial BMI around 8% in both groups, we verified a decrease on visceral fat, fasting glucose, triglycerides, leptin and TNF-α. Only orlistat group (which was more insulin resistant at baseline) presented significant reduction of glycemia post-overload, area under curve and insulin sensitivity index, beyond a larger percentual reduction of CRP. We detected a significant adiponectin reduction in orlistat group. Conclusions: Our findings point that weight loss around 8% on individuals with a higher degree of insulin resistance results in a more evident improvement on inflammatory state and insulin sensitivity, which ocurred independently of adiponectin and TNF-α changes
