RESUMO

The Medical Technology approach from monitoring examinations, counting lymphocyte cells (CD4) and quantifying HIV plasma viral load (Carga Viral Plasmática-CVP), has made an important advance in the early diagnosis of immunological and viral failure in patients living with HIV/Aids. This approach along with antiretroviral medication are contributing factors to changing a Syndrome changing of a condition that has inevitably taken the individual with HIV/Aids to an early death to a chronic condition that has somehow maintained the quality of life of patients with HIV/Aids. The assistant physicians meanings and practices with patients living with Aids were understood in the context of physician-patienttechnology, the later understood here only as CD4 and CVP blood exams. The methodology of this study was a qualitative analysis of semi-structured interviews as well as fieldwork. Twenty seven patients and four pysicians were interviewed. The fieldwork took place at two CTA in South Bahia. The thematic analysis identified categories that run over in speeches. It was found that the physician-patient relationship was based on hegemonic medicine. Nevertheless, an anamnesis and a detailed clinical examination were not part of the physicians main technological valise during their medical consultation. Instead, the blood exams were the major role, which should have had only an auxiliary role. Professionals insisted in the interpretation of blood work in order to find a response to patients? behavior, treatment efficacy, compliance to treatment regimen, disease objectivity, as well as criteria for anticipating an opportunistic disease and/or a terminal disease. The physicians? speech overused the blood exam interpretations and sometimes didn?t take into account patients subjective voice. In view of the patients? need of having their subjective problems considered, the physicians have referred them to other professionals. According to these doctors, indicating a therapeutic procedure, evaluating treatment efficacy, and the definitive caring of patients to weaken the virus as well as maintaining patients immunity preserved, were all expressed as their major purpose/responsibility in CTA. To patients, the blood exams translated the disease as a metaphor of viral military confrontation against CD4 cells. Furthermore, the patients expressed that the exam results were like a warning about an emergency of an unexpected event, an opportunistic disease or even a death warning which has created a condition of the expectations and anxiety. The findings also showed that to physicians and patients, the exams configured the ?disease face and represented the main point of consultation. This approach reinforced the hegemonic biomedical model and the meaning of physicians? voice by patients. Therefore, this study suggests that biomedical paradigm be overcome and also stimulates physicians to learn the science of humanities during their interaction with patients. Also, this study recommends the recognition of light technologies communicational useful to physician-patient relationship as a way for a humanized and integral health care.
