Estudo metabolico e resposta hemodinamica a sobrecarga oral de glicose em individuos portadores de hipertensão arterial essencial

RESUMO

To determine the possible relationship between insulin resistance, essencial hypertension and rriyocardium hipertrophy we studied 25 hypertensive patients (14 whites and 11 blacks), and 20 nOl:motensi ve (10 blacks and 10 whites). Secondary forms of hypertension were excluded as well other intercurrent illnesses, like DMNID and obesity. The voluntaries were submitted to an insulin tolerance test (0,1 U/kg of human insulin - TTI), and to an oral glucose tolerance test (OGTT) , concomitantly with a repeated echocardiographic evaluation (30 min intervals) . It was observed: - Seven hypertensives (about 30%) are insulin resistant (TTI) - Six hypertensive are hyperinsulinemicby the criterion of Total area under the insulin curve. These patients, alI with normal glucose tolerance, had higher glucose levels at 60 min after glucose ingestion. - Twel ve (12) hypertensives had rriyocardium hipertrophy, 45% of them were insulin resistant and only 10% were hyperinsulinemic. - A cardiac inotropic response was seen in alI, normotensive and hypertensive subjects, earlier. and stronger in the hypertensives. The blood pressure and periferic vascular resistance decreased in all groups, but it was higher in hypertensives and in nornlotensive blacks then in whites, even in basal period. There lS no difference on hemodinamics effects between patients with or without myocardium hipertrophy. - The double cross index decreased only in hypertensives till 30 or 60 minutes after glucose ingestion. There is no change of cardiac rate during all the experiments. The data obtained suggest that insulin resistance does develop in a subset of hypertensive patients, that the normotensive blacks are more" insulin resistant" than the whi tes . And also that hypertrophic patients there is no significant differences on their hemodinamic response. Also there is 45% of insulin resistant subjects in thes subgroup. there is a significant difference between whites and blacks concerning on their hemodinamic changes related to a glucose ingestion. An important point is that between hypertensive hypertrophic patients there is no significant differences on their hemodinamic response. Also there is 45% of insulin resistant subjects in these subgroup
