Estados pre-leucemicos na infancia : estudo clinico e hematologico

RESUMO

A retrospective evaluation of 18 pre-leukemia in children was made among 320 patients that were referred to the Pediatric Department at A.C.Camargo Hospital in São Paulo - between January 84- December 91 with previously diagnostic of leukemia to be confirmed. Lymphoblastic leukemia was confirmed in 227 patients and non lymphoblastic leukemia in 54. Among the 18 patients that were classified as pre-leukemia, 8 had myelodysplastic syndrome, 7 chronic myeloid leukemia and 3 developed ALL after pancitopenia. The MDS patients was classified acording to the FAB criteria and 5 were RAEB, 1 MMoCL and 1 case was secundary MDS. A score was done with the peripheral blood counts and with the bone marrow slides. Were analysed and compared with the ALL, ANLL, CML and aplastic disease. The clinical manifestations,the liver and spleen enlargement, the survival and follow up was also studied. The morphological findings among the MDS patients was similar with adults patients, as mentioned in the literature, but the incidence is lesser. The RAEB type was more frequent and more agressive disease. Some cases with pancytopenia simulating aplastic anemia developed ALL. This situation is different from the hipocelular MDS were the patients in these cases developed AML latter
