Cistite : fatores de virulencia da Escherichia coli em sua patogenese

RESUMO
This study investigated the correlation between the local mucosal response and the virulence factors of Escherichia coli strains isolated from the urine of 159 female patients with clinical symtoms of acute cystitis, Based on clinical analyses the patients had either leukocyturia (BL), leukocyturia and hematuria (BLH) or no of leukocyturia (B). The virulence factors studied in the Eco/i strains isolated include the expression of types 1, P and S fimbriae and of Dr hemagglutinin as well as the production of hemolysin (Hly) and cytotoxic necrotizing factor (CNFl). None of the Eco/i strains had S fimbriae and Dr hemagglutinin was detected in only three, out of 21 strains tested The presence of strains expressing type 1 fimbriae .and producing cytotoxins (Hly and CNF 1) was not correlated with the local mucosal response. Ecoli strains with P fimbriae were most frequently observed in patients up to 49 years old These individuais also had leukocyturia. The absence of a clinical vesical response was more prevalent in patients more than 50 years old There was not always a direct correlation between the presence of the different themselves such that the expression of one factor did not necessarily imply that of a second. This relationship also depended on the patient s age thereby allowing us to classify those up to 49 years old as having the following phenotype: type 1 fimbria +.. Hly +.. CNF 1 + and P fimbria~. Of the hemolytic Eco/i strains (Hly +), 63,8% were CNFl+ while in the CNFl+ strains, 90,2% were Hly +. Fifty percent of the strains with P fimbria, were also hemolytic. The most common serogroup was 06. In the P fimbriae positive strains 83,3% belonged to one of the eight most prevalent serogroups presenting in the urinary trdçt infections of these, serogroup 07 was the most common
