Blood contamination and cleaning substances influence on adhesion / Influência da contaminação com sangue e de diferentes agentes de limpeza na adesão de um sistema autocondicionante aos tecidos dentais

RESUMO
Although new self-etching adhesive systems provide a faster application due to a reduced number of components and application steps, the risk of contamination by oral fluids is not eliminated. Achieving good moisture control is a common problem in restorative dentistry. Blood contamination can occur especially when rubber dam isolation is not feasible, like carious lesions found near to gingival margin. To simulate gingival bleeding in laboratory study, some investigators have used freshly capillary blood, on the other hand other investigators have used venous blood sample with an anticoagulant. Among the studies that used venous blood with anticoagulant, some authors used this blood immediately after. However, other authors used the collected blood sample within one week. Based on this variety of blood manipulation, the aim of the present study was to investigate, in the first phase, the influence of storage period of the heparinized blood on microshear bond strength of a 1-step self-etching adhesive system to enamel and dentin. In second phase, the influence of heparinized venous blood vs. fresh capillary blood on microshear bond strength of a 1-step self-etching adhesive system to enamel and dentin was tested. In the third phase of this study the efficiency of different cleaning agents (Dakins solution, Tergentol and Hidrogen peroxide) on microshear bond strength of a 1-step self-etching adhesive system to enamel and dentin contaminated by blood was investigated. The results show that heparinized blood can be used to simulate gingival bleeding in laboratory study for up to 7 days without difference in its deleterious influence on adhesion tests. Contamination with fresh blood or heparinized blood resulted in lower values of adhesion, compared to enamel and dentin without contamination. All cleaning agents used in this study were efficient to recover the lost microshear bond strengths of blood contaminated enamel and dentin.
