Avaliação retrospectiva de um programa odontologico no protocolo pre-operatorio do transplante de figado

RESUMO

Introduction: The oral cavity of a liver transplant patient who undergoes liver transplant needs proper evaluation before surgery in order to eliminate infectious diseases that could impair the transplant. Objective: To assess the oral cavity of patients with liver disease and to investigate its association with the degree of liver disease. Methods: 257 patients (70 female, 187 male), selected for liver transplantation by the Unit of Liver Transplantation/HC-UNICAMP had their oral cavity status assessed by clinical and X-ray examinations at the Dentistry Unit of CECOM/UNICAMP. Results: Hepatitis C positive response was detected in 140 (54.47%) patients and hepatitis B in 122 (53.04%). Alcoholism antecedence was reported by 122 (53.04%) patients. The mean value for RNI was 1.74 (1-9) and for platelets the number was 104.470/mm³ (11.000-620.000). 228 (88.72%) patients needed odontological treatment. Periodontal disease was observed in 149 (64.22%) patients, with higher incidence in those patients with alcoholism antecedence and active caries lesions in 167 (71.98%) of the patients. 61.97% of the patients who had acrylic odontological protheses also presented oral candidiasis, reflecting poor hygiene procedures. Conclusion: The above findings indicate the relevance of the elimination of infectious disease in the oral cavity in patients with liver disease who will be submitted to liver transplantation. Other systemic diseases, such as the deficiency in the coagulation of the patients, reflect the need for good cooperation between the odontological and medical teams
