Avaliação da estimulação elétrica transcutânea no modo burst nos sintomas dolorosos de mulheres com endometriose pélvica

RESUMO

The pelvic endometriosis is a gynecologic disease that is often associated to dysmenorrhea, chronic pelvic pain and dyspareunia. In spite of varius treatments for the pain control, the transcutaneous electrical nerve stimulation (TENS) may be a proposal of analgesia for these women for presenting good results in other condition of chronic pain. The goals of this study are: determination the efficacy of the TENS in the control of painful symptomatology in women with pelvic endometriosis; verify the incidence of trigger and tender points in women with endometriosis, localizing them and correlating with the pain intensity; evaluation the use of TENS according the stage of the pelvic endometriosis. The study was randomized, prospective and blind with 42 women with endometriosis proved by laparoscopy and biopsy, without associated diseases and distributed in: the TENS group (n=22) and the control group (n=20). The TENS group was submitted to electrical stimulation protocol (mode burst), during 30 min for 27 sessions; in the control group of women were submitted to the use of TENS through the same protocol, but without passage of therapeutic electrical stimulation (placebo form). The pain was measured a visual numeric scale (VNS) before and after the each session for twenty-seven sessions and after thirty days after the end experimental of the protocol; it was applied the McGill s questionnaire at the beginning of the sessions (before), after the twentyseventh session (during) and after thirty days (after). The localization of the trigger and tender points was obtained the use of through algometer. Statistical analysis was performed by the test of Mann-Whitney, by the Friedman s test and in the dichotomous variable by the Fisher s test. It had been observed a significant reduction of the pain before and after each application of TENS, at the end of 27 sessions and after thirty days from the end of treatment (p <0,04, p <0,001 e p <0,05 respectively) of TENS group with relation to the control group; it pointed out a reduction of the number and of the localization of the triggers e tender points (p =0,004) of TENS group with relation to the control group; It didn t present relevant differences in the reduction of perception of painful symptomatology in initial stage (I/II) in comparison with advanced stage (III/IV) during and after the use of TENS (p = 0,39 e p = 0,69, respectively) in the TENS group. The results indicated that technique for TENS is effective the treatment and in the immediate control of pelvic pain in women with pelvic endometriosis and in the reduction of the number of the trigger and tender points.
