Avaliação da disfunção gonadal e tratamento com metformina do hipogonadismo masculino associado à síndrome metabólica

RESUMO

The Metabolic syndrome is among the most common metabolic disorders nowadays. In the gonadotrophic axis, it can be associated with the state of male hypogonadism. The objectives. were evaluate the incidence of hypogonadism in males with metabolic syndrome and its correlation with insulin, then evaluate the effect of the treatment with an insulin sensitizer (metformin) on the gonadal function. The first part of the study was analytical, retrospective and transversal. The selected patients were stratified into two groups according to levels of total testosterone: Group 1: 300 ng / dL (normal gonadal function) and group 2: <300 ng / dL (hypogonadic). The second part was an analytical, prospective and interventional study, of the kind study case in male patients with metabolic syndrome. Of the 80 males evaluated, 30% presented low concentration of total testosterone (group 2). This group, when compared to group 1, presented higher IMC values, waist and hip circumference, insulin, Homa-IR, Homa- and triglycerides, presenting, however, lower SHBG and free testosterone values. Inverse correlations of the insulin with total testosterone and SHBG and of the Homa-IR with total testosterone were observed. The treatment with metformin was effective in increasing concentrations of total and free testosterone and in reducing the concentrations of insulin and HOMA IR in both groups. The incidence of male hypogonadism associated with the metabolic syndrome is sensible and can be a sign of worse metabolic profile with accentuated resistance to insulin. Therefore, it should always be investigated in patients with metabolic syndrome. The use of metformin in the treatment of male hypogonadism is promising and further intervention studies are needed for its analysis.
