AvaliaÃÃo do escore de Alvarado no diagnÃstico de apendicite aguda em crianÃas e adolescentes no Instituto Materno Infantil de Pernambuco (IMIP)

RESUMO

The aim of this study was to assess the accuracy of the Alvarado score in the diagnosis of acute appendicitis among children and teenagers at Instituto Materno Infantil de Pernambuco (IMIP). It was conducted a validation study of a diagnostic tool (laboratorial-clinical score to acute appendicitis diagnosis) having the histopathology test of the appendix used as gold standard. The sample was composed by eighty one children and teenagers who were admitted at the Pediatric Emergency Unit of IMIP with abdominal pain suspected of acute appendicitis from March to October 2002. The patients were submitted to Alvarado score at Pediatric Emergency Unit and the decision to operate on was made by another surgeon. Taking the cutoff point of >7 points at Alvarado score, it was found a sensitivity of 72,2%, specificity of 86,4%, positive predictive value (PPV) of 92,9% and negative predictive value (NPV) of 55,9%. Reducing the cutoff point to >6 points the sensitivity was 81,5%, specificity of 72,7%, PPV of 88% and NPV of 61,5%. Finally taking the cutoff point of >5 points it was found a sensitivity of 81,5%, specificity of 72,7%, PPV of 88% and NPV of 55,9%. The Alvarado score is simple, fast, reproductible and taking the cutoff point of >5 points, it has a high value as a diagnostic screening tool for children and tennagers with suspected acute appendicitis
