RESUMO

Introduction: hand hygiene is regarded as the most important method of prevention of healthcare-associated infections. The hand hygiene adherence can be influenced by several factors, which must be identified in order to implement rewarding or corrective measures. Objectives: our objectives were: to evaluate the hand hygiene adherence in a pediatric oncology hospital; to analyze the factors related to hand hygiene compliance; to evaluate the appropriateness of glove use; to measure knowledge of healthcare workers regarding the importance of hand hygiene; to examine the impact of the interventions. Material and Methods: we conducted a prospective study in three phases between July, 2006 and December, 2008. The first phase consisted in an observational study of hand hygiene adherence. In the second period we implemented some interventions with the aim of improving the hand hygiene adherence, including a survey distributed to healthcare workers, and the development of activities for encouraging this attitude, based on the analysis of the data from the first phase and the survey. Finally, the third phase corresponded to an observational period. Results: the hand hygiene adherence was considered low, however we observed improvement after the intervention (39,01% in phase 1 to 52,17% in phase 3). The healthcare workers preferred to wash their hands with water and soap, especially after the patient care, and the use of the alcohol hand gel has been improved in phase 3. The highest hand hygiene adherence rate has been observed among the physicians, in the bone marrow transplantation unit, when the healthcare worker washed their hands with water and anti-septic soap, during the patient care in contact precautions and when the healthcare workers worn gloves. The results of survey showed that healthcare workers possessed good knowledge concerning the importance of the adequate hand hygiene.
