RESUMO

Public Health Nursings challenge when working in primary health care is to incorporate the concept of generation as a category to identify needs and contribute to the organization of the health practices, mainly the ones in terms of child assistance on the perspective of integrality and bigger solvability. The incorporation of Health Promotion in the planning of practices in primary health care can contribute to health services improvement since it expands the object of the disease to the health needs of the groups. For this reason, it is necessary to recognize primary health care nursings practices and conceptions related to Health Promotion. This study analyzed the recommended intervention approaches in official documents through the main actions and normalized activities and identified the actions in child health from the perception of professional nurses from assistance programs. It was done a qualitative research based on historical and dialectical materialism from the documentary analysis of child care propositions in the city of São Paulo and semi-structured interviews with nurses from the Butantã Health Technical Supervision. The documentary analysis focused on three documents in which was carried out a survey about the main points of action that it is designated and analyzed according to Labontes (1996) referential scheme. The semi-structured interviews were performed with eleven nurses, from traditional primary care units, family health units and mixed primary care unit, organized according to the Collective Subject Discourse methodology and analyzed through the hermeneutic-dialectic reference. The documentary analysis results suggest a greater importance of the physical dimension compared to the other dimensions and interfaces of Health Promotion. The interviews analyses indicate that the Health Promotion concept is understood as preventive actions aiming at specific worsening and risk groups and the life style transformation. This vagueness leads to other difficulties in assessing and overcoming problems, restricting professionals to obtain Health Promotion in their practice. The primary health care mentioned practices rely on the assistance programs recommended by the State, standing out the epidemiological surveillance activities, which are an essential pivot. However, the structuring of the actions directed to the extended concept of Health Promotion should embrace actions between sections and directed at the individual behavior, as well as to strengthen the person as a social performer. We conclude that the organization of the actions in child health in primary health care is close to the conception of Caplans (1993) traditional-functionalist model, in which health problems´ sources are incorrect habits and life style. However, when the generation category is incorporated in the nursing practices planning, child health assistance can have its main point changed to the promotional and preventive perspective, without excluding individual and curative care, so the child would be seen inserted in a familiar and community environment
